Priorities of Care app for iPhone and iPad

“The Priorities of care are a touchstone for every point of care in the last days of life, for everyone from patients and their loved ones to frontline health and care staff , commissioners and regulators”

The App was developed by the Palliative Care Team of Airedale NHS Foundation Trust to help clinical staff in the delivery of the 5 Priorities of Care.
The 5 Priorities of Care, were developed by the Leadership Alliance for Care of Dying People, to help focus on the needs and wishes of the dying person and those closes to them.
